The Adult Case Manager is responsible for delivering Community Psychiatric Support Treatment (CPST) services to adults age 18 and older with complex and multiple service needs. This role includes developing collaborative, goal-oriented treatment plans and supporting clients in achieving identified outcomes. The position requires effective time management, the ability to work independently in community-based settings, and the capacity to collaborate within a multidisciplinary team to ensure coordinated, client-centered care. The Adult Case Manager works under the direct reporting of the Assistant Managers of Traditional Case Management and in guidance / collaboration with other PHS Leaders.
