About The Position

Siena Francis House is committed to welcoming, sheltering, and empowering individuals experiencing homelessness to navigate their path toward safe and stable housing. The Veteran Case Manager provides intensive, community-based case management services to Veterans transitioning from homelessness into permanent housing through Department of Veterans Affairs (VA) Grant and Per Diem (GPD) and Healthcare for Homeless Veterans (HCHV) programs. Using a Critical Time Intervention (CTI), Housing First, and trauma-informed approach, this position supports Veterans in achieving long-term housing stability, increasing self-sufficiency, and connecting to healthcare and community resources. This position works extensively in the community and independently manages participant engagement, service coordination, documentation, and housing stabilization activities throughout Omaha and Lincoln.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, or related field.
  • Minimum of two years of experience providing case management, housing stabilization, or supportive services.
  • Experience coordinating mainstream benefits and community resources.
  • Valid driver’s license, proof of insurance, reliable transportation, and an insurable driving record.
  • Ability to work independently while maintaining strong collaboration with team members and partner agencies.
  • Strong organizational, communication, and documentation skills.
  • Ability to effectively engage and communicate with individuals from diverse socioeconomic, racial, ethnic, and cultural backgrounds.
  • Demonstrated commitment to serving individuals experiencing homelessness through compassionate, participant-centered practices.
  • Proficiency with Microsoft Office applications including Outlook, Word, and Excel.
  • Ability to learn and effectively utilize HMIS and other electronic documentation systems.

Nice To Haves

  • Master’s degree in Social Work or related field.
  • Experience working with Veterans or Veteran-serving systems.
  • Knowledge of Critical Time Intervention (CTI) and Housing First models.
  • Familiarity with VA programs and homeless service systems in Omaha and Lincoln.
  • Experience working within HMIS or other case management databases.

Responsibilities

  • Provide intensive case management services to Veterans enrolled in VA GPD and HCHV programs.
  • Assist Veterans with housing stabilization, benefits coordination, healthcare access, employment resources, financial management, and community integration.
  • Conduct regular home and community visits to monitor housing stability, assess participant needs, and support progress toward individualized goals.
  • Develop individualized service plans using participant-centered, strength-based, and trauma-informed practices.
  • Coordinate referrals and linkage to services including: mental health treatment, substance use treatment, medical care, employment services, financial assistance, supportive community resources.
  • Maintain accurate and timely case documentation, data collection, and reporting requirements in HMIS and other required systems.
  • Participate in case conferencing and collaborative planning with VA programs, Continuums of Care, landlords, and community service providers.
  • Support program quality and compliance with grant requirements, agency policies, and best practices.
  • Assist with identifying service trends, program outcomes, and opportunities for program improvement.
  • Provide education and coaching related to: tenant responsibilities, budgeting and financial literacy, transportation access, meal planning, community resource navigation, development of natural support systems.
  • Utilize de-escalation techniques and sound professional judgment in unpredictable or high-stress situations.
  • Perform other duties as assigned.
