About The Position

The Case Manager will be responsible for working with eligible Veteran families to obtain/retain housing, develop housing stability plans and assist in obtaining eligible program supportive and temporary financial services. The Case Manager will also routinely market and conduct program outreach activities.

  • Must be willing to travel the service area as needed.
  • Agency policy regarding drug and alcohol testing and criminal background checks applies to this position.

Nice To Haves

  • Bachelor’s degree preferred.
  • Prior experience in case management, housing-centered programs or Veteran affairs also preferred.

Responsibilities

  • Assess participant needs to determine what program supportive and/or temporary financial services are required to stabilize the household.
  • Provide potential client referrals in a timely manner to the Outreach and Eligibility Specialist so they may fully screen for program eligibility.
  • Intake/Enroll program applicants once case has been assigned by Assistant Project Director.
  • Develop a housing stability plan that includes how participants will fund their housing, utilities, food, etc. after program financial assistance ends.
  • Assess housing units to ensure adequacy for participants and compliance with Department of Housing and Urban Development guidelines.
  • Work with the Housing Coordinator to establish and maintain positive relationships with local landlords/property owners in order to provide housing for clients.
  • Submit check requests and applicable documentation to the Assistant Project for payment processing in Blackbaud.
  • Maintain client data in the Homeless Management Information Systems database as well as EmpowOR.
  • Conduct recertification reviews every 90 days of each client household to ensure eligibility for continued services.
  • Conduct periodic follow-up assessments of clients’ housing status after services have ended.
  • Submit charts for review to Assistant Project Director and/or Project Director at initial enrollment, after each 90-day recertification, and upon exit.
  • Establish and maintain positive relationships with other human service agencies and private business owners that may provide supportive and/or temporary financial services to clients.
  • Assist in designing marketing/outreach materials such as emails, letters, flyers, etc. outlining eligibility and services of the program.
  • Market and conduct program outreach activities on a routine basis to the Veteran community and establish a relationship to potential employers, public assistance programs/providers and the VA Benefits Office.
  • Perform other duties as assigned by supervisor.
