About The Position

Avenues Recovery Center is hiring a Case Manager for its inpatient detox and residential treatment facility in Fort Collins, Colorado. The Case Manager supports clients throughout treatment by advocating for them, addressing barriers to care, coordinating resources, and planning for discharge. Responsibilities begin at intake and continue through the duration of treatment and post-discharge planning.

Requirements

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.
• Excellent communication, multitasking, and time-management skills.
• Strong problem-solving and critical-thinking skills.
• Warm, empathetic, and respectful personality.

Nice To Haves

• High school diploma or GED.
• At least one year of experience in the treatment industry.

Responsibilities

• Advocate for clients and address and resolve external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange post-discharge resources as needed, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
