At North Valley, the Case Manager I supports adults receiving mental health services by helping them access resources, develop skills, and achieve their individualized recovery goals. Working closely with clients and the treatment team, this role provides targeted case management and psychosocial rehabilitative services using evidence-based practices. Through advocacy, coordination, and ongoing support, the Case Manager I helps individuals build stability, increase independence, and improve their overall quality of life. The Case Manager I is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities and ensuring that regulatory compliance and organizational standards are met.
