About The Position

The Case Manager I supports adults receiving mental health services by helping them access resources, develop skills, and achieve their individualized recovery goals. Working closely with clients and the treatment team, this role provides targeted case management and psychosocial rehabilitative services using evidence-based practices. Through advocacy, coordination, and ongoing support, the Case Manager I helps individuals build stability, increase independence, and improve their overall quality of life.

Requirements

• High School diploma or equivalent
• None - see Preferred Qualifications
• Case Manager certification (post hire as needed)
• CPR certification (post hire as needed)
• Valley de-escalation certification (post hire)
• Driving positions require a minimum age of 21 and a current driver’s license

Nice To Haves

• One year social services or behavioral health experience
• Bachelor’s degree in Social Services, Psychology, or related field

Responsibilities

• Provides targeted case management services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Observes and collects client urine samples as necessary
• Transports clients to appointments as necessary
