About The Position

Acenda Integrated Health provides over 100 behavioral health programs, including mental health services, crisis care, substance use recovery, family therapy, maternal and pediatric support, and residential programs across New Jersey. The Projects for Assistance in Transition from Homelessness (PATH) is an outreach intensive case management program designed to support individuals with serious mental illness who are experiencing or at risk of homelessness. The BRIDGE program is a community-based outreach program serving adults 18 and over in Gloucester County with a serious mental illness and legal justice involvement, supporting community integration by connecting individuals to resources that promote stability, independence, and ongoing support.

Requirements

  • Possess a Bachelor of Arts in a behavioral health field from an accredited institution and one (1) year of relevant, post degree experience; or possess an Associate’s degree in a direct care field (e.g. psychosocial rehabilitation or psychiatric nursing) from an accredited institution and two (2) years of relevant post degree experience; or possess a minimum of a high school degree, have the ability to use his/her lived experience of recovery from mental illness, and either possess or complete a nationally or NJ approved peer certification training program within one year of hire.
  • Must have and maintain a valid driver's license with an acceptable driving record.
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Responsibilities

  • Engages consumers in the discharge planning process to assist with the transition to the community through probation/jail.
  • Provides post-discharge follow-up to support the consumer with community integration through direct in-home contact and linkage to community resources.
  • Links consumers to resources that support the ability to remain stable within the community.
  • Conducts assessments within correctional facilities and attends court hearings to advocate for persons served.
  • On call as needed for client crisis needs.
  • Provides weekly face-to-face contact with clients in the community by pre-scheduling sessions and utilizing treatment plan goals for session preparation.
  • Provides linkages across needs, warm hands, and follow up for resources to ensure client stability.
  • Transports clients for identified goal needs and stabilization for successful linkage needs.
  • Maintains all documentation as identified in state contract and program manual.
  • Monitors treatment planning ongoing and discharge planning.
  • On-site presence at jail and consistent communication with probation and other community agencies for referrals and pre-discharge planning.
  • Participates in all program, agency, supervision meetings as required.
  • Maintains yearly required case management trainings.
  • Completes all required documentation: notes within 24 hours of contact, as well as treatment plans and assessments as documented in program manual to support timely billing and required timeframes.
  • Adheres to a flexible schedule based on client and program needs.
  • Demonstrates competent written and verbal communication with referral sources and external customers.
  • Attentive and responsive to customer needs in a helpful, supportive and timely manner.
  • Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve.
  • Meets obligations of the program (contractual, licensing, Medicaid, etc.).
  • Follows program policies and procedures and encourages the same in others.
  • Educates self on issues impacting the program population.
  • Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies).
  • Performs other related duties as assigned.
