About The Position
Case management positions consist of advocating and assisting participants with waiver services.
Requirements
- BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences
- One year of experience working with ID/DD individuals (ex: volunteering or having a relationship with family or friend who may have ID/DD)
- OR one year of case management experience
- Valid Driver’s license and vehicle insurance required
- Strong organizational skills
- Ability to multi-task
- Excellent communication skills
- Be self-motivated
- Be able to conduct and facilitate meetings
- The ability to collaborate with other team members
- Ability to advocate for the needs of participant
- Must be able to operate a motor vehicle and have reliable transportation with valid insurance
- Must provide verbal and communication with team members
Responsibilities
- Conduct annual assessment and person-centered service plan
- Conduct monthly face to face visits with participants
- Assist clients with accessing community resources
- Monitor participants progress with waiver services including health, safety and welfare in monthly contact note
- Assist with building a network of supports
- Assure participant choice and access to community resources
- Return phone calls and emails promptly
- Complete and submit documentation by required deadlines
- Perform other related work as required and assigned
