About The Position

Greystone Foster Care, in partnership with Universal Health Services, Inc., is seeking a dedicated and compassionate Case Manager to join their growing team. This is a hybrid role, blending remote work with in-office responsibilities and occasional facility reporting. The position requires full-time dedication and flexibility to work evenings and weekends. The Case Manager will be responsible for ensuring compliance with state regulations and the Greystone Foster Care plan, completing all placement documentation accurately and on time, conducting regular file audits, assisting with foster parent reassessments, scheduling and documenting child/family team meetings, visiting assigned children as needed, ensuring timely scheduling of annual EPSDT and biannual dental appointments, arranging outpatient mental health assessments for Level 2 youth, and obtaining necessary consents for medical appointments and medications.

Requirements

• Bachelor's degree in Social Work or related field
• One to three years' related experience and/or training, or equivalent combination of education and experience
• Valid state Driver's License
• Automobile insurance with verification of medical liability
• Must reside in the Southern MIDDLE TN area of the state: McMinnville, Manchester, Tullahoma, Winchester, Lebanon, and Mt. Juliet.

Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan
• Ensures that all placement documentation is completed according to established timelines
• Completes regular file audits for each child and participates in regular peer file audits to ensure audit preparedness at all times
• Assists with completing foster parent reassessments when necessary
• Schedules child/family team meetings at least quarterly and corresponding documentation is completed/submitted
• Visit each assigned child per policy or as needed
• Ensures that annual EPSDT and biannual dental appointments are scheduled and information provided to DCS
• Ensures that outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs
• Obtains required consents and releases from appointed guardians for medical appointments and medications
