This role serves as the primary liaison for assigned Children’s Crisis Residence, collaborating with government agencies, community organizations, external service providers, and family members. The Case Manager is responsible for exchanging information, identifying needs, and coordinating activities, including the intake and discharge of youth, and resolving concerns between parties. The position involves receiving referrals, reviewing application materials, conducting in-person intake meetings (which may require travel across New York City), and developing professional, empathetic relationships with families and service providers. The Case Manager advocates for the youth, fosters communication, and facilitates admissions by developing person-centered, trauma-informed individual support (ISP) and crisis plans. They are responsible for creating and maintaining case files, ensuring all documents are obtained and updated, and maintaining regular contact with families to support continuity of care. Collaboration with the multidisciplinary team is crucial to ensure optimal care, and the Case Manager must collect and maintain all required documentation and reports in appropriate systems. A key responsibility is leading discharge planning, including identifying referral sources, arranging post-discharge services, and conducting follow-ups. The role also involves establishing and maintaining linkages with community organizations and providers to support youth transitions and engaging with the Mental Health Clinician and Program Supervisor for support and communication. Compliance with all Federal, State, Local, and regulatory agency requirements, including HIPAA, is mandatory. The position may also involve providing coverage for other roles as needed.
