About The Position

Case Managers work alongside clients to identify what matters most to them and build the skills, resources, and confidence to get there. Through individualized support and psychosocial rehabilitation, you’ll help clients overcome barriers, strengthen independence, and turn personal goals into meaningful progress. Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Requirements

• High School diploma or equivalent
• None (Experience)
• Case Manager certification (Post-hire as needed — required within 120 days of start date)
• CPR certification (Post-hire)
• Valley de-escalation certification (Post-hire)
• Driving positions require a minimum age of 21 and a current driver’s license

Responsibilities

• Provides psychosocial rehabilitative services as described by the Medicaid manual
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley’s standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Transports clients to appointments as necessary
