About The Position
As a Case Manager, you'll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes. You'll facilitate transitions to the next level of care, manage critical diagnoses, and optimize health outcomes across the care continuum, making a direct impact on patient wellbeing.
Requirements
- Bachelor’s degree in nursing or Master’s degree in Social Work.
- Minimum 3 years of clinical experience.
- Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW).
- Excellent verbal communication, written documentation, and interpersonal skills.
- Strong problem-solving, analytical, and decision-making abilities.
- Proficiency in computer skills and knowledge of healthcare systems.
Nice To Haves
- Certification in Case Management (CCM) or Accredited Case Manager (ACM).
- Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
Responsibilities
- Collaborate with interdisciplinary teams to assess patient needs and develop individualized care plans.
- Coordinate discharge planning to ensure safe transitions to the next level of care.
- Advocate for patients and families by connecting them with community resources and support services.
- Evaluate care outcomes and adjust strategies to optimize health and wellness.
- Promote patient safety, quality of care, and cost-effective solutions across the care continuum.
