About The Position

Devereux Advanced Behavioral Health is seeking a Case Manager for Webster House in Webster, MA. This role involves working with children in need and making a difference in their lives. Webster House is an off-campus group home serving co-ed youth, ages 6 to 12, on a rapid reintegration care program. The program emphasizes identifying and treating safety/risk factors, barrier behaviors, and factors that precipitated placement, with the goal of facilitating reintegration of the youth to their family or next appropriate placement in the shortest time period possible. The position offers opportunities for growth within a human services agency.

Requirements

  • Must be 21 years of age or older.
  • Valid driver's license required.
  • High School Diploma with three years of congregate care experience or Bachelor’s degree with one year of congregate care experience required.
  • Must be able to attend a 2-week paid orientation, Monday to Friday, 8AM to 4PM, in Rutland, MA.

Nice To Haves

  • Degrees in the subject area of psychology, social work, sociology, or related field preferred.

Responsibilities

  • Work closely with the Program Manager and the Clinician to provide case management services as a liaison between center, families, agencies, social workers, lawyers and other involved parties.
  • Complete QI responsibilities for programs as well as gather clinical information and complete documentation as assigned.
  • Serve as a member of individual treatment teams, program milieu team, and QI team.
  • Provide and ensure therapeutic interventions in a compassionate and safe environment.
  • Engage with youth in a meaningful and positive manner, teaching and prompting appropriate behaviors, and providing positive reinforcement for the same.
  • Teach and assist with Activities of Daily (ADL) Living Skills.
  • Possess adequate organizational and communication skills needed to complete required documentation, such as progress notes and incident reports.
