Case Manager ( RN )
About The Position
We are seeking a skilled Registered Nurse Case Manager to assess patient needs, coordinate care, and support effective communication among everyone involved in patient care. The role includes coordinating nursing care, home health aide services, and applicable therapies after approval by the Department of Labor. The Case Manager must protect client and office records in accordance with HIPAA guidelines and the Privacy Act PHI, and collaborate with the care team to provide cohesive, comprehensive care. Requirements
- Valid and active Registered Nurse license in good standing.
- At least one year of experience in a home health setting or related area, with strong assessment skills.
- Excellent organizational and time-management skills to manage multiple clients efficiently.
- Proficiency with computer and internet-based applications and office equipment.
- Competent working knowledge of client-based equipment.
- Must pass a criminal background check and sanction screening.
- Professional demeanor and appearance.
Responsibilities
