About The Position

We are seeking a skilled Registered Nurse Case Manager to assess patient needs, coordinate care, and ensure effective communication among all parties involved in patient care. The role supports energy workers and their families through care under the Energy Employees Occupational Illness Compensation Program Act (EEOICPA). The company has over 14 years of experience and operates across 24 states.

Requirements

• Hold a valid and active Registered Nurse license in good standing.
• Have at least one year of experience in a home health setting or related area, with strong assessment skills.
• Have excellent organizational and time management skills to handle multiple clients efficiently.
• Be proficient in computer and internet-based applications, as well as office equipment.
• Have competent working knowledge of client-based equipment.
• Pass a criminal background check and sanction screening.
• Maintain a professional demeanor and appearance.

Responsibilities

• Oversee the coordination of nursing care, home health aide services, and applicable therapies once approved by the Department of Labor.
• Maintain confidentiality of all client and office records in accordance with HIPAA guidelines and the Privacy Act PHI.
• Ensure effective communication and collaboration among the care team to provide cohesive and comprehensive care.
• Perform other duties as assigned to support the team and enhance patient care.
