About The Position

We have an exciting opportunity for a Case Manager to join our multidisciplinary Team within Albany Med Center of Excellence for Alzheimer's Disease to make a significant regional impact on delivering high quality specialized care for patients with Alzheimer’s disease and other related dementias. Under the general supervision of Albany Medical College, Department of Neurology and Manager of the Alzheimer’s Program, the Case Manager will play a vital role in the clinical operations of the Center of Excellence for Alzheimer’s Disease (CEAD) at our Community Neurology Division. The Case Manager will be responsible for all social work activities associated with patient clinical visits providing comprehensive assessment, evaluation, and ongoing support.

Requirements

• Master’s degree in social work or allied field
• Proficiency in the use of Microsoft Suite Programs
• Strong organizational skills
• Ability to communicate effectively both verbally and in writing
• Ability to manage and prioritize multiple assignments
• Ability to work well with others and independently

Responsibilities

• Participates in patient visits as needed.
• Responsible for reporting the patient history and complaints to the provider.
• Maintains confidentiality and adheres to HIPAA regulations.
• Administers baseline and routine cognitive testing (MMSE, MoCA) as well as ADL/IADL, CDR neuropsychological inventory, AD8, as indicated.
• Provides counseling and case management services to patients and/or care partners to help them understand and accept their diagnosis, cope with the emotional stress, and manage the day-to-day services.
• Facilitates community service referrals if/when needed and complete appropriate paperwork that is requested for services.
