Requirements

Bachelor's degree required.
Strong written and verbal communication skills.
Ability to maintain accurate documentation and electronic records.
Critical thinking and problem-solving skills.
Ability to complete required trainings, including CPR/First Aid, Medication Administration, Crisis Intervention, Trauma-Informed Care, HIPAA, Mandated Reporting, and related certifications.
Ability to lift up to 25 pounds and meet the physical requirements of the position.

Responsibilities

Provide therapeutic case management services to adolescents and their families.
Coordinate medical care, treatment planning, and service delivery.
Ensure residents receive the support and resources needed to succeed.
Maintain residents' medical records and ensure regulatory compliance.
Schedule and coordinate routine, specialty, psychiatric, and follow-up appointments.
Assist with transportation to medical appointments when needed.
Monitor medication administration and complete weekly medication counts.
Train staff in medication administration, CPR, and First Aid.
Communicate medical updates with parents, guardians, and treatment team members.
Maintain Individual Service Plans (ISPs) in compliance with licensing standards.
Schedule and track ISP meetings, goals, and documentation within the Electronic Health Record (EHR).
Notify families and treatment team members of upcoming ISP meetings.
Support behavioral goal development and mentor direct care staff.
Participate in and help facilitate ISP meetings.
Work closely with clinicians, direct care staff, families, and community providers.
Assist with program operations as needed, including providing coverage for Youth Behavioral Specialists and Supervisors.
Complete additional duties that support quality care and program success.
