About The Position

The Service Coordination program is currently seeking a part-time Case Manager who has a passion for working with children and adults with intellectual and developmental disabilities and helping them reach their desired goals. Our team takes a holistic approach, focusing not just on the individual with the disability, but also on their family and community!

Requirements

  • Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field
  • Two years of experience in a human services field
  • An equivalent combination of training and experience
  • Current valid driver’s license and good driving record required
  • Based on the Virginia DMV point system, records must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months
  • Out-of-state driving records must be obtained by applicant and presented at time of interview
  • Records must reflect at least three years of history and be dated within thirty days of interview date
  • Pre-employment drug testing, FBI criminal background check and education/degree verification required
  • Working knowledge of case management methods, practices, and procedures
  • Ability to interview clients to identify deficiencies in personal, social, economic and health needs
  • Ability to work in a multi-disciplinary team environment providing treatment and rehabilitation services
  • Ability to communicate effectively orally and in writing
  • Ability to operate a personal computer and related software and other standard office equipment
  • Ability to develop and maintain effective working relationships with internal and external customers

Nice To Haves

  • Two (2) years of direct service to those with Intellectual Disability (ID) and Developmental Disability (DD) diagnosis preferred
  • Qualified Developmental Disabilities Professional (QDDP) designation preferred
  • Those candidates with verbal and written fluency in both English and Spanish are encouraged to apply
  • Knowledge of various development disabilities, their impacts, and best practices for support including the Developmental Disability (DD) Waiver System preferred
  • Experience with an electronic health record preferred

Responsibilities

  • Assists the Service Coordination team with documentation needs, resource connection, and related administrative duties to support case managers with meeting the person-centered needs of individuals on their caseloads
  • Connects to and monitors community services for children and adults with intellectual and developmental disabilities
  • Acts as an advocate for individuals ensuring that their rights are upheld and that they receive the services and supports they need
  • Conducts comprehensive assessments to understand the individual's needs and strengths; develops personalized service plans that outline goals and necessary supports
  • Provides ongoing support to individuals and their families, including education about available services and strategies for navigating the system
  • Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence)
  • Performs other work as required
