Requirements

• A Bachelor's Degree in a mental health-related field from an accredited institution
• Two years of relevant experience
• Strong knowledge of community resources and local agencies
• A valid driver's license with an MVR that meets agency standards
• Clear Five year driving record/abstract with ability to be insured by organizations carrier.

Nice To Haves

• Experience advocating for supportive services
• Spanish-speaking ability

Responsibilities

• Building relationships with clients and assisting them in navigating housing options
• Connecting individuals to entitlements, benefits, and supportive services
• Conducting community outreach to identify people who may qualify for PATH services
• Collaborating with service providers, treatment teams, and natural supports
• Participating in the development of Individual Recovery Plans (IRPs)
• Providing interventions that promote empowerment, independence, and self-advocacy
• Using Motivational Interviewing (MI) to effectively engage and support clients
