About The Position

EAC Network, a leading not-for-profit social service agency, is seeking a dedicated full-time Case Manager to join the Safe Harbour Program. This role supports the Children & Youth Population by providing case management and direct services for minor victims of human trafficking and youth at-risk of human trafficking. The Case Manager will be part of the Suffolk County Child Advocacy Center’s Multi-Disciplinary Team and will coordinate with other agencies.

Requirements

  • Bachelor's Degree in Mental Health Counseling, Social Work, Forensic Psychology or Behavioral Health field required.
  • Minimum of two years of relevant experience.
  • Proficient in computer applications or other automated systems such as Google Drive, spreadsheets, Microsoft Office applications, calendar, and email and/or database software.
  • Excellent communication, organizational and interpersonal skills.
  • Must have access to automobile and possess valid NYS driver’s license.

Responsibilities

  • Complete risk-assessments for minor victims of human trafficking and youth at-risk of human trafficking.
  • Provide trauma informed, victim centered case management services and outreach to minor victims of human trafficking and youth at-risk of human trafficking in Suffolk County, NY.
  • Conduct assessments for client needs and develop an individualized service plan for safety planning, goal setting, and psychoeducation.
  • Document services provided in the CAC NYSCA database.
  • Collaborate with community partner agencies to deliver staff training on identifying unaccompanied minors at risk of trafficking and connecting them to services.
  • Transport and escort clients to court proceedings, medical appointments, and other appointments.
  • Assist with applying for public benefits, employment, medical care, return of personal property, and coordination of services.
  • Make appropriate referrals for medical exams, mental health services, and residential assistance.
  • Maintain an accurate database of professional referral resources.
  • Prepare monthly and quarterly statistical and annual reports as required by funding source.
  • Attend workshops and trainings as needed for project implementation.
  • Assist in preparing reports for the grant as needed.
  • Participate in team meetings, case conferences, agency presentations, trainings, and related events.
  • Perform all other relevant duties as determined by Supervision and EAC NETWORK Administration.
