About The Position

The Case Manager is responsible for coordinating services and supports for children and adults with intellectual and developmental disabilities through the CHOICES Waiver and CTS programs. This position works directly with individuals, families, providers, and community partners to ensure participants receive person-centered services that promote independence, community integration, and quality of life. The Case Manager serves as an advocate, facilitator, and resource coordinator, helping individuals access medical, behavioral health, educational, vocational, housing, transportation, recreational, legal, and other community-based supports. This role requires strong organizational skills, excellent communication, and a commitment to person-centered planning. This position offers the ability to work remotely; however, regular travel within the assigned service area is required.

Requirements

  • Bachelor's Degree; OR Equivalent combination of education and relevant experience.
  • Experience in case management, social services, human services, healthcare, behavioral health, or developmental disabilities preferred.
  • Experience working with individuals with intellectual and developmental disabilities is preferred.
  • Strong understanding of person-centered planning principles.
  • Knowledge of community resources and service systems.
  • Excellent communication, interpersonal, and advocacy skills.
  • Ability to work independently and manage competing priorities.
  • Strong organizational and time-management skills.
  • Ability to maintain confidentiality and comply with HIPAA requirements.
  • Intermediate computer skills and proficiency with electronic documentation systems.
  • Valid driver's license.
  • Reliable transportation.
  • Ability to travel regularly throughout the assigned service area.
  • Ability to establish and maintain a HIPAA-compliant home office.
  • Reliable internet connectivity for remote work responsibilities.

Responsibilities

  • Coordinate services and supports for individuals enrolled in CHOICES Waiver and CTS programs.
  • Assist individuals in identifying goals, preferences, strengths, and support needs.
  • Facilitate development and implementation of Individual Service Plans (ISPs).
  • Support individuals in directing and actively participating in their person-centered planning process.
  • Monitor implementation of service plans and intervene when issues or barriers arise.
  • Conduct ongoing assessments to identify changing support needs.
  • Link individuals and families to community resources, programs, and services.
  • Advocate for access to appropriate supports and services.
  • Assist participants in navigating healthcare, behavioral health, educational, vocational, housing, transportation, and social service systems.
  • Promote self-determination, choice, and independence for individuals receiving services.
  • Participate as an active member of a multidisciplinary team.
  • Collaborate with individuals, families, providers, clinical staff, medical professionals, and community agencies.
  • Participate in recovery planning and service reviews.
  • Maintain professional relationships with community organizations and service providers.
  • Monitor service delivery and provider performance to ensure services meet identified needs.
  • Complete required documentation and maintain accurate case records.
  • Enter data and maintain records in Therap and other required systems.
  • Review and report Critical Incident Reports (CIRs) in accordance with program policies.
  • Participate in Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews, as appropriate.
  • Ensure compliance with program requirements, state regulations, and organizational policies.
  • Utilize Person-Centered Thinking and Planning principles.
  • Apply the Charting the LifeCourse framework when supporting individuals and families.
  • Focus on achieving personal outcomes and improving quality of life.
  • Evaluate effectiveness of services and recommend adjustments when needed.
