About The Position

The Case Manager is responsible for ensuring efficient and effective delivery of the highest quality transitional and support services for clients. They will provide support services to clients to receive appropriate services, and case files are maintained according to contract requirements. Services to clients are intended to aid them in improving their social, economic, cultural, or educational status. Responsibilities include providing intake functions, coordination of program activities; development and implementation of specialized programming such as job placement and retention, family budgeting, enforcement of policies, and guidelines set by the program.

Requirements

Bachelor’s Degree in Social Work or another human service field with relevant coursework.
Experience in case management and budgeting is essential.
Must be committed to the elimination of racism, sexism, classism, homophobia, and other forms of oppression.
Working knowledge of local County area resources for client assistance is required, including low-income housing.
Must have an appreciation and understanding of the dynamics of homelessness and a willingness to assist those who are experiencing homelessness.
Must have experience in technology-based office management and office support.
Proficient in Microsoft Word; working knowledge of PowerPoint, Excel, and Outlook.
Must be able to work well with various personalities within the Catholic Church and be comfortable within a large, complex, and diverse working environment.
Requires an appreciation and respect for the Catholic Church and its teachings. Church employees must conduct themselves in a manner that is consistent with and supportive of the mission and purpose of the Church. Their public behavior must not violate the faith, morals or laws of the Church or the policies of the diocese.

Maintain appropriate case files in accordance with HIPAA and other utilized program requirements and guidelines.
Perform all intake functions; beginning files for each client.
Maintain timely records using the database designated by the agency to track demographics following guidelines for participant intake.
Review applications with the Clinical Services Team to make a decision regarding the client’s eligibility.
Ensure that all clients apply for available subsidies.
Assist the client in setting goals and identifying their immediate needs.
Provide information, referral, and problem-solving assistance to residents.
Conduct regular meetings with program participants.
Monitor the achievement of service goals.
In conjunction with the Program Director, provide program management, development, and expansion in conjunction with the agency's strategic plan.
Assist in marketing and developing the program.
Work with volunteers assigned to the program in conjunction with the Catholic Charities Volunteer Coordinator.
Maintain current knowledge regarding literature, trends, developments, and policy changes affecting the homeless population.
Produce timely reports as required.
Attend and participate in all meetings and training as required by the agency.
Work as a liaison with other agencies, churches, businesses, and individuals in the community to coordinate services.
Assist staff/programs in emergency/crisis situations as directed.
Assure compliance with any grant contracts.
Monitor program quality and convey corrective measures need to the Program Director.
Compile accurate statistical information.
Maintain standards of accountability for all financial transactions.
Participate in Agency continuous quality improvement (CPQI) program.
Other duties as assigned.
