About The Position

The Case Manager provides case management, classification, and transitional services to inmates/residents in an institutional, secure setting. The role supports individuals’ social involvement, health needs, and community life, and coordinates programming and resources to prepare them for transition or release into the community. The Case Manager may serve as the point of contact in the Unit Manager’s absence, when applicable.

Requirements

• A bachelor’s degree from an accredited college or university; additional experience in a criminal justice field or in case management services may substitute for the education requirement on a year-for-year basis, up to two years.
• Ability to communicate clearly verbally and in writing.
• Experience with Microsoft Office applications or similar software applications.
• A valid driver’s license.
• Must be at least 21 years of age.

Nice To Haves

• Experience providing case management services is preferred.

Responsibilities

• Participate in the intake process by collecting, organizing, and analyzing information about individuals.
• Provide direct casework services related to inmates’/residents’ social involvement, health needs, and community life.
• Prepare and collaborate on reports and develop case presentations for assigned inmates/residents.
• Coordinate appropriate programming, resources, and services for social development and preparation for transition or release into the community.
• Serve as the point of contact in the Unit Manager’s absence, when applicable.
