The Assisted Living Program (ALP) helps people disabled due to HIV/AIDS who are homeless. ALP helps individuals obtain a housing subsidy, secure an apartment and then provides ongoing case management to help each individual maintain their health, housing and independence. This role is responsible for providing direction and support to program efforts to ensure the safety of persons served and families, staff, visitors, and division sites. The Case Manager will also ensure staff are trained in safety initiatives and that procedures are followed, monitor safety drills, oversee agency safety initiatives, and create methods for implementation. They will follow procedures on hand washing, universal precautions, COVID/infection control, and OSHA regulations, and ensure staff compliance. The role involves delivering functions in various appropriate settings, including driving personal vehicles. Responsibilities include following through with reports and investigations for critical incidents, providing direct supervision or assigning supervision duties, leading meetings, participating in hiring processes, and attending division-wide meetings. The Case Manager will support a positive workplace culture, participate in Performance Improvement (PI) functions, establish relationships with community partners, operate within the approved budget, and complete other duties as assigned by their supervisor.
