About The Position

Senior Solutions (Area Agency on Aging in Southeastern Vermont) is seeking a Case Manager to work with older adults and/or people with disabilities in the White River Junction/Windsor and the surrounding areas. This role utilizes a person-centered planning approach and requires a deep understanding of state and federal benefits. Strong organizational, communication, and computer skills are essential. The ideal candidate is dedicated, caring, and passionate about making a positive impact on the lives of seniors and individuals with disabilities, eager to learn about benefits and services for aging Vermonters.

Requirements

• Bachelor’s degree in human services and prior case management experience preferred.
• A combination of relevant education and experience may be substituted for a degree.
• Must have reliable transportation and a valid driver's license.

Responsibilities

• Collaborating with other community partners to create a person-centered plan of care.
• Monitoring and reassessing program service plans as required.
• Having a working knowledge of services and programs available to older Vermonters and people with disabilities.
• Assisting, when necessary, to gain access to existing services such as public benefits, housing, legal services, etc.
• Ensuring that all provided services do not exceed budgeted amounts and are consistent with DAIL guidelines.
