About The Position

Rehabilitation Hospital of Southern New Mexico is seeking a per diem Case Manager to join its patient-care team in Las Cruces, New Mexico. Requirements

One year of recent relevant experience is required; experience in an acute care setting is preferred.
Current clinical licensure is strongly preferred.
Understand treatment costs and financial support in relation to quality and efficiency.
Demonstrate critical thinking and the ability to evaluate alternative solutions, conclusions, or approaches.
Use appropriate leadership skills to delegate, organize, and educate coworkers and staff.
Have knowledge of community resources for patients’ post-discharge clinical and social needs.
Have knowledge of clinical operations and procedures.
Establish and maintain proactive relationships with interdisciplinary team members, local payers, physicians, community service organizations, and healthcare facilities.
Maintain quality, safety, and/or infection control standards.
Demonstrate general computer skills, including data entry, word processing, email, and record management.
Have effective organizational and time-management skills.
Have effective written and verbal communication skills.
Maintain appropriate confidentiality.
Work closely and professionally with others.

Responsibilities

Work with physicians and multidisciplinary team members to develop plans of care for assigned patients.
Monitor patient care through assessments and/or patient records to ensure progress toward desired outcomes.
Identify and resolve barriers that hinder effective patient care.
Actively participate in the discharge planning process.
Integrate the hospital’s mission and Guiding Principles into daily practice.
