About The Position

This person will be a member of the Recovery team of WestCare Tennessee and will primarily be responsible for working collaboratively with the Treatment Counselor, Supportive Employment Coach and Peer Support Specialist in the Supportive Employment grant. The Case Manager will provide services to individuals and meet the needs of the individual to determine appropriate referrals.

Requirements

  • Graduation with a four-year degree in Addiction Counseling, Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or related field is required.
  • 2 – 4 years of experience in related project field may be substituted for degree.
  • Case management experience preferred.
  • Ability to work collaboratively with others in a professional manner.
  • Ability to work well in a team environment.
  • Ability to exercise sound judgment and discretion.
  • Ability to operate a computer and complete documents in Microsoft Word and Excel program formats.
  • Ability to respond as needed to a crisis in an efficient manner.
  • Ability to complete work tasks within scheduled work hours.
  • Possess excellent verbal and written communication skills.

Responsibilities

  • Embrace and embody the mission, vision, guiding principles, clinical vision and goals of WestCare Tennessee.
  • Deliver evidence-based curriculum to individuals in a group setting.
  • Educate and refer individuals to community agencies and businesses that appropriately address the individual needs for food, shelter, clothing, employment, transportation, and other community resources.
  • Assist individuals with establishing or re-establishing support systems within their community.
  • Advocate on behalf of the individual in order for them to receive access to needed services in addressing appropriate needs.
  • Participate in staffing decisions regarding individuals’ current enrollment status in order to help them to meet the goals of their Individual Service Plan (ISP) and treatment plan.
  • Complete documentation of services on provided forms promptly.
  • Provide input to the Treatment Counselor regarding the individual treatment needs which may include conducting discharges, incident reports, and other related documentation.
  • Perform individual follow up interviews to document individual progress, to make additional referrals if needed and to maintain active communication throughout the duration of their enrollment in the program.
  • Engage and complete the required WestCare Tennessee eLearning, and any other program related trainings.
