About The Position

The Chinese-American Planning Council, Inc. (CPC) is seeking a Case Manager for the Learn and Earn Program at Franklin Delano High School in Brooklyn, NY. This program aims to provide work readiness skills training, college access support, leadership development, and employment opportunities for youth and young adults. The Case Manager will help students cope with various barriers to success, implement behavior plans, provide case management, refer students and parents to resources, document progress, and develop workshops. The role also involves building a network of worksites for internships and collaborating with partner organizations. CPC is the nation's largest Asian American social services agency, dedicated to the social and economic empowerment of Chinese American, immigrant, and low-income communities.

Requirements

  • Bachelor's in Social Work, Education, Counseling, Psychology, Youth Studies or related field
  • At least 2 years of leadership experience in the youth development field and/or previous experience working with High School aged-youth
  • Strong work ethic and demonstrated passion for working with immigrant and low-income communities, and youth development
  • Excellent communication, organizational and interpersonal skills; has knowledge of youth and immigrant issues
  • Knowledge of MS office applications and virtual platforms such as Zoom, Teams, and Webex

Nice To Haves

  • Masters preferred
  • Bilingual in Spanish, Chinese and/or another language a plus

Responsibilities

  • Help students cope with disabilities, behavior management, mental health issues and other potential barriers to their success
  • Provide intervention for students with significant social, emotional and/or behavioral needs
  • Implement individual behavior/growth plans and provide case management services
  • Refers students and parents to appropriate support resources, monitor and accelerate the progress of the student and parents referred
  • Document participant case notes, track progress, and maintain program files
  • Develop and facilitate interactive workshops related to career exploration, work readiness, higher education, financial literacy, leadership development, and other life skills
  • Build a network of worksites for summer internship opportunities
  • Collaborate with partner organizations to enhance programming
  • Fulfill other duties as assigned by the Program Director
