About The Position

IPMG is seeking a full-time Case Manager for Boone County. This role is hybrid, requiring travel for community-based meetings, and the work setting is remote (home office). The Case Manager will provide information and education on the Medicaid waiver process, create Individual Support Plans based on the strengths, needs, goals, and desires of the individuals served, help individuals access necessary services and supports, and facilitate regular community-based team meetings to review the plan of care and collaborate with service providers.

Requirements

  • A valid driver's license, car insurance, and reliable transportation.
  • A Bachelor’s Degree in Human Services and a minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities.
  • Designated home office setting with computer, internet service, and smartphone.

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process.
  • Create an Individual Support Plan based on the strengths, needs, goals, and desires of the Individuals we serve.
  • Help Individuals access services and supports they need to meet their goals.
  • Facilitate regular community-based team meetings to review the plan of care and collaborate with service providers as needed.
