About The Position

IPMG is seeking a full-time Case Manager for Jennings County. This hybrid position requires travel to community-based meetings multiple times per week and allows for remote work from a home office. Applicants must reside within the State of Indiana or within 10 miles of the state line due to the requirement for in-person meetings.

Requirements

  • A valid driver's license, car insurance, and reliable transportation.
  • A Bachelor’s Degree in Human Services.
  • A minimum of one year of professional experience working with Individuals with intellectual and developmental disabilities.
  • Designated home office setting with computer, internet service, and smart phone.

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process.
  • Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served.
  • Help Individuals access services and supports they need to meet their goals.
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
