About The Position

IPMG is seeking a full-time Case Manager for the Porter County H&W/TBI Waiver program. This hybrid role involves remote work from a home office combined with community-based travel for meetings. Candidates must reside in Indiana or within 10 miles of the state line to accommodate in-person meetings. IPMG is a 100% employee-owned company through an Employee Stock Ownership Plan (ESOP), offering shared success and growth opportunities.

Requirements

• A valid driver's license, car insurance, and reliable transportation
• RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
