Requirements

  • Ability to pass DHS background check
  • Driver’s license, access to a reliable vehicle, vehicle insurance & a clean driving record

Responsibilities

  • Serve a caseload of clients (average of 38 clients per caseload)
  • Complete new client intakes
  • Develop initial service plan with the client and legal representative
  • Coordinate services between providers and clients
  • Schedule in-person meetings with clients at least twice per year
  • Make monthly phone contact with each client
  • Maintain quality documentation, such as case notes, service plans and service agreements
  • Work to uncover client needs and aspirations
  • Connect clients to appropriate services and resources
  • Act as a liaison for the client to ensure their whole care team is well aligned
