About The Position

IPMG is offering a sign-on bonus of $500 within the first 6 months. IPMG is 100% employee-owned through an Employee Stock Ownership Plan (ESOP), meaning every Employee-Owner shares in the company’s success and growth. This full-time, hybrid position combines remote work from your home office with travel of up to one hour for community-based meetings several times a week. Candidates must reside within Indiana or within 10 miles of the state line to accommodate required in-person meetings.

Requirements

  • A valid driver's license, car insurance, and reliable transportation
  • RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
  • The ability to pass thorough background screens
  • Designated home office setting with computer, internet service, and smart phone

Responsibilities

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed
