The Case Manager serves as the primary client-centered service professional responsible for guiding families from homelessness to housing stability and long-term independence. This position provides trauma-informed, goal-driven case management services that address housing, employment, education, financial stability, childcare, transportation, and behavioral health barriers. The Case Manager develops individualized service plans, monitors measurable progress, and ensures compliance with HUD, Continuum of Care (CoC), and Providence House standards. Through structured accountability, coordinated referrals, and consistent follow-up, this role directly supports sustainable housing outcomes and family self-sufficiency.
