About The Position

The Case Manager provides direct support to clients by assessing their needs, developing service plans, connecting them with community resources, and monitoring progress toward self-sufficiency. Working in a non-profit setting, the Case Manager serves as an advocate, counselor, and liaison to ensure clients receive the services and support necessary to overcome barriers and achieve their personal, social, and economic goals.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s preferred in some programs).
  • Experience in case management, social services, or community-based work (non-profit experience preferred).
  • Strong knowledge of local resources and service systems (housing, healthcare, benefits, employment, etc.).
  • Excellent communication, advocacy, and problem-solving skills.
  • Ability to manage multiple cases, meet deadlines, and maintain confidentiality.
  • Cultural competence and sensitivity when working with diverse populations.
  • Proficiency with Microsoft Office and case management databases.

Responsibilities

  • Conduct intake interviews and needs assessments with clients to identify strengths, challenges, and goals.
  • Develop and implement individualized service or treatment plans.
  • Provide ongoing case management, including referrals to housing, healthcare, employment, education, financial assistance, and social services.
  • Advocate for clients within community systems and assist in navigating complex services.
  • Maintain regular contact with clients to monitor progress and provide support.
  • Collaborate with community partners, government agencies, and internal staff to coordinate holistic care.
  • Maintain accurate and timely case notes, records, and data in compliance with agency and funder requirements.
  • Facilitate support groups, workshops, or life-skills training as needed.
  • Respond to crises and provide short-term intervention and problem-solving support.
  • Track client outcomes and prepare reports for program evaluation and funding compliance.
