Case Manager - FT | Texarkana LTACH
About The Position
The Case Manager coordinates care across disciplines for a designated patient population. The role reviews inpatient admission records to assess resource utilization and whether admission is appropriate for the level of care, identifies discharge needs, and coordinates discharge planning. The Case Manager works with the interdisciplinary team to support desired financial and quality outcomes and maintains and enhances relationships with payors. The position is full-time at PAM Health’s Texarkana LTACH. Eligible professional disciplines may include occupational therapy, physical therapy, registered nursing, respiratory therapy, speech therapy, or licensed social work.
Requirements
- Current BLS certification.
- Three to five years of inpatient experience.
- Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
Nice To Haves
- State licensure where the hospital is located is preferred.
- Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Responsibilities

- Assess and identify discharge needs.
- Coordinate an appropriate discharge plan.
- Collaborate with the interdisciplinary team to facilitate desired financial and quality outcomes.
