About The Position

Merakey is seeking a Case Manager to join their Assertive Community Treatment (ACT) team in Harrisburg, PA, supporting Dauphin County. The ACT team provides comprehensive services including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are delivered in the individual's home or community, with at least three home visits per week. The Case Manager will assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses, support treatment plans, provide crisis intervention, collaborate with families and community resources, ensure client safety in residential programs, and participate in direct care and the treatment planning process. Personal vehicles are required for client transportation and community-based work.

Requirements

• Bachelor's Degree in a human service field required.
• 1 year experience working with individuals in a recovery setting and or crisis experience.
• Valid PA Driver's License.
• Travel is necessary to offer support across Dauphin County.
• Must utilize personal vehicles for client transportation and community-based work.

Responsibilities

• Assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses.
• Support treatment plans, including substance abuse services, education, and family consultation, under clinical supervision.
• Provide crisis intervention services as needed.
• Collaborate with families to offer education, support, and guidance.
• Collaborate with various community resources, including healthcare providers, social services, housing agencies, and other support organizations, to ensure comprehensive care for clients.
• Ensure the safety and well-being of consumers in residential programs by adhering to safety protocols.
• Participate in direct care responsibilities, including daily living support and therapeutic activities.
• Follow and contribute to the treatment planning process in collaboration with the clinical team.
