About The Position

The Case Manager is a vital member of a patient-centered care team, responsible for coordinating and advocating for patients and their families. The role identifies and assesses patient needs, develops care strategies that promote safety, quality, and cost-effective outcomes, facilitates transitions to the next level of care, manages critical diagnoses, and supports health outcomes across the care continuum. The position is with Henry Ford Wyandotte Hospital, serving the Downriver community as part of Henry Ford Health.

Requirements

• Bachelor's degree in nursing or master's degree in social work.
• Minimum 3 years of clinical experience.
• Valid State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW).
• Excellent verbal communication, written documentation, and interpersonal skills.
• Strong problem-solving, analytical, and decision-making abilities.
• Proficiency with computer systems and healthcare information technology.

Nice To Haves

• Certification in Case Management (CCM) or Accredited Case Manager (ACM).
• Experience in discharge planning, home health care, rehabilitative medicine, or managed care.

Responsibilities

• Collaborate with interdisciplinary teams to assess patient needs and develop individualized care plans.
• Coordinate discharge planning to ensure safe transitions to the next level of care.
• Advocate for patients and families by connecting them with community resources and support services.
• Evaluate care outcomes and adjust strategies to optimize health and wellness.
• Promote patient safety, quality of care, and cost-effective solutions across the care continuum.
