We are seeking a passionate and mission-driven Case Manager to help expand the impact of Shepherd’s House Ministries across all sites and advance this life-changing work. The Case Manager is accountable for providing consistent, trauma-informed, participant-centered support that promotes safety, stability, and progress toward housing and personal well-being. This role is responsible for engaging participants, assessing needs, developing and monitoring individualized plans, coordinating access to appropriate resources and services, maintaining accurate and confidential documentation, and collaborating effectively with staff, volunteers, and community partners. The Case Manager upholds organizational values, ethical standards, and compliance requirements while contributing to a safe, respectful, and healing environment across all service settings.
