Adult Outpatient Programs provide the least restrictive level of care, offering adults clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization, and/or preventing movement to a higher level of care. Treatment varies according to the consumer’s presenting symptoms. Case Managers monitor consumers’ well-being and needs related to mental health, physical health, living, learning, working, and social environments. They travel to consumers’ homes, boarding homes, and other community-based living settings to provide monitoring. The primary role is to improve consumers’ psychological adjustment by empowering consumers, and sometimes family members, with information, education, and support to follow through after case management ends. The role includes clinical case management, assessment and service planning, community-resource linkage, coordination with internal services and treatment-team members, discharge planning, clinical documentation, caseload management, and participation in supervision, training, and organizational activities. The position also involves the physical demands described in the posting.
