About The Position

The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to ensure appropriate utilization of hospital resources and determine whether admissions meet the appropriate level of care. The Case Manager assesses and identifies discharge needs, coordinates suitable discharge plans, and works collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The role also maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience; experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.

Nice To Haves

Licensure in the state where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
