About The Position

This position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives. The role involves obtaining in-depth social histories, developing and assuring community and/or agency resources to meet the consumer's needs, and serving as a Qualified Intellectual Disabilities Professional (QIDP) while providing monthly monitoring of the program. This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines. Travel is extensive, and the employee must possess a valid driver's license and maintain required car insurance.

Requirements

  • Bachelor's degree in a human services field (any degree from an accredited college or university except for a degree in a physical, natural or biological science or mathematics).
  • One (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field (for Level I).
  • Two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field (for Level II).
  • Possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of experience working directly with individuals with intellectual and/or developmental disabilities (for Level I).
  • Possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of experience working directly with individuals with intellectual and/or developmental disabilities (for Level II).
  • Completion of the Case Manager Academy Training Program.
  • Valid driver's license.
  • Required car insurance.
