The Case Manager provides essential case management services to individuals receiving day or employment services through the Developmental Disabilities Administration (DDA) and/or Division of Rehabilitation Services (DORS). This role involves coordinating with community resources, public benefits, and natural supports to meet the assessed needs of the individuals supported. The Case Manager assists individuals in becoming eligible for public benefits and other resources, while meticulously maintaining files and records. A key aspect of the role is coordinating and facilitating Person-Centered Planning meetings, ensuring adherence to all required timelines and tracking the Person-Centered Plan (PCP) through its approval process. The Case Manager also prepares comprehensive social service summaries and reviews medical assessments, acting as an advocate for the individuals supported and intervening in crisis situations. Additionally, the role includes coordinating transportation, attending various meetings, utilizing agency electronic systems, and ensuring proper communication through email. This is a full-time position requiring onsite presence.
