The COIN Clinic Case Manager is responsible for reducing barriers to care for patients with complex medical and social needs, including those living with HIV/AIDS. This role supports patients in navigating healthcare systems, accessing treatment, and connecting to essential social services such as housing, benefits, and mental health support. The Case Manager empowers patients with the tools, knowledge, and resources needed to advocate for themselves, while serving as a key liaison between patients, providers, and community-based organizations to ensure coordinated, comprehensive care. This is a grant-funded position.
