About The Position

We are seeking a Registered Nurse Case Manager to assess patient needs, coordinate care, and support effective communication among everyone involved in patient care. The role includes coordinating nursing care, home health aide services, and applicable therapies after approval by the Department of Labor. The Case Manager must protect confidential client and office records in accordance with HIPAA guidelines and the Privacy Act PHI, and collaborate with the care team to provide cohesive, comprehensive care. Other duties may be assigned to support the team and enhance patient care.

Requirements

• Hold a valid, active Registered Nurse license in good standing.
• Have at least one year of experience in a home health setting or related area, with strong assessment skills.
• Demonstrate excellent organizational and time-management skills to manage multiple clients.
• Be proficient with computer and internet-based applications and office equipment.
• Have competent working knowledge of client-based equipment.
• Pass a criminal background check and sanction screening.
• Maintain a professional demeanor and appearance.
