About The Position

Mission Action is seeking a Case Manager for its DSP program. This role involves completing client intake assessments, data entry, conducting wellness checks, and providing care coordination, crisis intervention, and conflict resolution. The Case Manager will work collaboratively with Mission Action staff and city staff to ensure effective site operations. The position also includes other organizational duties such as adhering to the Mission Action Employee Handbook, working within the agency's mission and values, and treating community members with respect and dignity.

Requirements

  • Demonstrated ability to perform all essential duties and responsibilities of the position
  • Strong word-processing and computer skills, including knowledge of Microsoft Word and Excel
  • Must be well organized, efficient, highly motivated, and able to handle multiple tasks at once, work well under pressure, and take initiative
  • Ability to work independently as well as within a team
  • Approach work from a trauma informed lens
  • Strong commitment to serving low-income and underserved communities

Nice To Haves

  • Fluency in Spanish strongly preferred, including ability to provide written translation of documents
  • Experience working with low-income, immigrant, and monolingual Spanish-speaking communities

Responsibilities

  • Complete client intake assessments and necessary documentation for all new clients of the site
  • Enter data daily into the RTZ system, OneSystem as per contract requirements
  • Report on Slack! And review site care coordination with site manager informed
  • Conduct client wellness checks to ensure the safety and well-being of all clients onsite
  • Provide care coordination, crisis intervention, and conflict resolution with clients
  • Work collaboratively with Mission Action staff and city staff to ensure effective coordination and operation of the site
  • Other duties as assigned
