About The Position

Under the supervision of the Program Director/designee, the Case Manager functions as part of a team supporting individuals living in independent apartments in the community. This role assists consumers in developing and maintaining stable living, working, and social environments by coordinating access to medical, psychosocial, financial, vocational, and other essential services. The Case Manager provides crisis intervention as needed and delivers services in the field to support individuals in achieving the highest level of independent living possible.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related field
  • Two (2) years of experience providing direct services to individuals with mental disabilities or related human services experience; OR Bachelor’s degree with a one-year practicum (e.g., BSW) and one (1) year of relevant experience
  • Must be authorized to work in the U.S.

Responsibilities

  • Travel to/visit consumers’ residences to provide supportive services on a schedule established by the program; counsel consumers individually or in groups to meet service needs and goals
  • Assist consumers in modifying attitudes and patterns of behavior by increasing understanding of self and others
  • Secure and evaluate information related to medical, psychological, and social factors; complete and maintain assessments, service plans, treatment records, and documentation (including quarterly reviews)
  • Maintain accurate, up-to-date case records and measurable service plans in compliance with regulatory standards and culturally responsive practices
  • Provide supportive counseling (individual and group) and utilize evidence-based approaches such as cognitive-behavioral techniques
  • Obtain, develop, and communicate resource information and connect consumers to appropriate community supports
  • Facilitate individualized services aligned with recovery and discharge planning goals
  • Educate consumers on available services, recreational activities, and daily living skills, including socialization and conflict resolution
  • Perform crisis assessments and provide verbal and non-verbal crisis intervention as needed
  • Refer consumers to appropriate community providers and accompany them to appointments
  • Participate in on-call responsibilities, if assigned
  • Report serious incidents immediately; complete incident documentation and follow agency policies
  • Plan and coordinate social and recreational activities
  • Conduct regular inspections of consumer residences to ensure safety and cleanliness
  • Maintain staff communication logs and documentation
  • Execute emergency response protocols and provide first aid/CPR if necessary
  • Attend meetings, supervision, and trainings
  • Advocate on behalf of consumers internally and externally
  • Assist consumers in accessing vocational training, employment, and education opportunities
  • Perform other job-related duties as assigned
