About The Position

Merakey is seeking a full-time Case Manager to join their Dual Diagnosis Treatment Team in their Dubois, PA location. The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.

Requirements

Bachelor’s degree in Social Work, Psychology, or another human service-related field required.
Will be required to complete clearances and background checks, at Merakey’s expense.
Valid PA driver’s license with a clean driving record required.
1 year of experience in the behavioral health field required.
Candidate must be open to community based travel.
