About The Position

The Taimon Booton Navigation Center (TBNC) is our shelter and service center for transgender, gender non conforming & intersex people, as well as cisgender women, to find sanctuary, community, and a pathway to housing. TBNC's gender supportive facility offers refuge, belonging, and safety and is the only shelter in Northern California attached to a Federally Qualified Health Center (FQHC). It is our hope that everyone who stays at TBNC leaves feeling safer, more supported, and on their way to stable housing. The Bilingual Case Manager, Level I, is responsible for providing housing needs assessments and linkage to the necessary resources to seamlessly move clients through the housing process. The case manager is also responsible for coordinating care plans with the client's providers, health education, supportive counseling, crisis intervention, benefits advocacy, and coordinating services within the program as assigned by their direct supervisor. Direct client work and interaction will be done both in the field and at the office.

Requirements

  • BA in a health sciences or social work related field or AAA/CMH/CHW OR comparable work experience
  • One year of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities
  • Knowledge and expertise of how to navigate the housing resources in San Francisco
  • Experience working with the LGBTQ community, specifically the Trans and GNC community
  • Knowledge serving individuals living with HIV /AIDS, STIs, substance use, mental health, social service, and probation system
  • Familiarity with issues in communities of color
  • Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.
  • Must be adaptive, flexible and a team player
  • Strong verbal and written proficiency in English
  • Experience working with diverse sexual/gender orientations, the homeless, limited English speakers, and/or multiple diagnosed individuals
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills
  • Familiarity and experience with Microsoft Word and Outlook programs
  • Detail oriented with strong follow through skills, including attention to and respect for detail
  • Must be able to sit, stand, and walk for extended periods of time
  • Some evening and weekend hours may be required.
  • Up to date with vaccinations, especially latest COVID 19 vaccine and seasonal flu shot vaccine
  • Proof of TB clearance (issued within the past 12 months or less)

Responsibilities

  • Conduct intake and assessment of new clients, with a specific focus on housing
  • Provide case management and advocacy services for clients
  • Provide housing education and advocacy, as well as general wellness education
  • Provide supportive counseling and develop service goals with clients
  • Conduct individual risk reduction counseling sessions
  • Provide information and referrals to external services and monitor referral completion
  • Complete necessary documentation, progress notes and treatment plans in a timely manner
  • Participate in case conferences, monthly staff meetings, and in service training
  • Participate in and comply with agency quality assurance, peer review, clinical consultation
  • Work with Care Services Team to identify new clients' needs and develop programs accordingly
  • Navigate and outreach clients as needed
  • Work on weekends and/or holidays as needed.
  • Perform other duties and tasks as needed
