About The Position

The Case Manager is responsible for implementing and monitoring Person-Centered Support Plans in day and residential programs. This role requires availability 24 hours a day when on call, with rotations for on-call duties lasting one week per month. The position involves working with individuals with intellectual and developmental disabilities to improve the quality of their lives.

Requirements

  • Bachelor’s degree required or can accept 2 years college coursework plus 6 years of experience with developmentally disabled (or 3 years college coursework plus 3 years of experience).
  • One year of experience working with persons with disabilities is required.
  • Valid TN driver's license and a good driving record.
  • High School Diploma or Equivalent.
  • US Work Authorization.
  • Must be at least 19 years of age.
  • 3 years of driving experience.
  • No more than 3 moving violations on your MVR within the past 3 years.
  • No prior felony convictions.

Nice To Haves

  • Preferred degree in Social Work, Sociology, Psychology, Special Education or related field.
  • Written and Verbal Bilingual Fluency is a plus.

Responsibilities

  • Implement and monitor Person-Centered Support Plans in day and residential programs.
  • Be available 24 hours a day when on call.
  • Learn and enforce all Sertoma Center, Inc. Personnel Policies and Procedures.
  • Learn and enforce all standards set forth by Licensure, Quality Enhancement, Medicaid Waiver, Fire Marshall, and the Center.
  • Serve as Officer for contract compliance in all areas.
  • Complete and monitor all paperwork requirements of the assigned caseload in a timely manner, including Person-Centered Support Plans, Behavior Support Plans, Staff Instructions, Monthly Service Reviews, and various assessments.
  • Coordinate, facilitate, document, and participate in all meetings pertaining to the assigned caseload and other required meetings.
  • Train all pertinent staff on individual plans, documentation, and required needs of the caseload.
  • Assist in communication between Residential Coordinators, direct care staff, ISC, and center staff.
  • Assist Director of Human Resources in interviewing, hiring, orienting, and training new staff.
  • Conduct unannounced visits as directed by the Chief Operating Officer.
  • Recognize that efforts are part of a team function and show consistent respect of co-workers, recognition of diversity, and practice an unbiased disposition.
  • Contribute to the mission and principles of the Center by being dedicated to serving individuals with disabilities and remaining alert to new concepts for improvement.
  • Utilize sound fiscal principles that stress cost containment by controlling overtime expense, turnover of direct care staff, reduction in replacement costs of home inventories, and dedication to safety and quality control.
  • Adhere to an acceptable level of professionalism to ensure the maintenance of confidentiality and discretion.
  • Observe all Center routine regarding work schedules, presence during working hours, break times, and lunch hours.
  • Strive to perform all work assignments and activities in an acceptable manner requiring minimal supervision.
  • Perform any other duties as assigned by supervisor or Chief Operating Officer.
